Add Data Source Information + Address Type to Curation Report (legal entity and address)

As a user I would like to know the data source of each record in the curation report, so that I can clearly identify a record in a system.

Additionaly, a BP can have multiple addresses that will be printed as separate lines in the report and are impossible to distinguish without looking at the data.

    Instead of just a job ID which is system generated it would indeed be of interest and good use if the data source(s) the report was generated from, would be listed somewhere. It is currently more or less guessing and remembering.
